At St. Michael’s Health Centre, therapy is about more than just movement. It’s about building connections that empower residents to live fuller, more independent lives. The long-term care therapy team plays a vital role in uniting families, residents and interdisciplinary teams to ensure that care is truly collaborative and resident-centered.
By embracing shared decision-making, they actively involve families in rehabilitation plans, ensuring each resident’s needs, goals and personal preferences are honored. Open scheduling allows families to participate in therapy sessions, keeping them informed and engaged in their loved one’s progress. This approach is especially meaningful for residents who are unable to advocate for themselves, as family input helps shape personalized treatment plans.
Strong communication is the foundation of their success. The team is highly visible in resident spaces, forming lasting connections with nursing care teams, allied health professionals, housekeeping and food services. These relationships create a seamless exchange of feedback, ensuring timely adjustments to equipment, treatment plans and overall care strategies. Their partnerships extend beyond the facility as well, collaborating with vendors, stakeholders and community resources to secure the best equipment and support for residents.
Their ability to work across disciplines strengthens care delivery. Whether teaming up with recreation staff to integrate physical activities into social events or coordinating with nursing to ensure equipment is used correctly, they amplify their impact through teamwork.
