The compassionate transitions team at St. Teresa Place goes above and beyond to ensure every resident feels safe, supported and valued as they navigate life’s changes.

When a new resident arrived at the site, unexpected challenges arose. Without local family or a support network, the transition was daunting. The team quickly stepped in, ensuring the resident’s room was ready and providing the emotional support needed during this difficult time. Their commitment extended beyond physical care, creating a welcoming space that fostered a sense of belonging.

Recognizing the resident’s struggles with basic necessities, the team worked swiftly to secure essential items, from bedding and clothing to storage solutions. A recreation therapist applied for a care grant, while a volunteer and clinical staff came together to provide interim supplies. Within 24 hours, the resident had everything they needed to settle in with dignity.

Through a non-judgmental and person-centered approach, the team honored the resident’s unique journey, responding with empathy and respect. Their actions reflect the very heart of Covenant’s mission offering compassionate care that goes beyond medical needs to embrace the whole person. By working seamlessly across departments and with external partners, the compassionate transitions team uses a model of care that prioritizes dignity, respect and human connection.
